Tumor / infection / inflammation – rare Pathophysiology • Compression → nerve root ischemia and inflammation • Leads to radicular pain, paresthesia, and motor deficits in the affected dermatome/myotome Clinical Features Pain • Radiates from neck to shoulder, arm, and hand • Follows dermatomal pattern Sensory Changes • Numbness, tingling, burning in affected dermatome Motor Deficits • Weakness in specific myotomes • Decreased reflexes in corresponding nerve root Common Cervical Nerve Root Patterns Examination • Spurling’s test: cervical extension + rotation → pain radiates along dermatome • Upper limb tension test / traction: may relieve symptoms • Motor, sensory, reflex assessment according to myotome/dermatome Investigations • MRI cervical spine: gold standard for nerve root compression • X-ray: may show spondylotic changes • EMG / Nerve conduction studies: differentiate radiculopathy from peripheral neuropathy Management Conservative (most cases) • Rest and activity modification • NSAIDs or short-term analgesics • Physical therapy: traction, posture correction, strengthening Interventional • Corticosteroid injection (epidural or selective nerve root) for persistent pain Surgical • Indicated for: severe or progressive motor deficits, intractable pain • Procedures: anterior cervical discectomy and fusion (ACDF) or foraminotomy • Pain radiates along dermatome of affected cervical nerve root • Sensory + motor + reflex deficits correspond to the root • C6 most commonly affected, followed by C7 • Spurling’s test positive → reproduces radicular pain • MRI = gold standard for diagnosis Summary Cervical radiculopathy is nerve root compression in the cervical spine, causing radiating neck and arm pain, sensory changes, and weakness in a dermatomal/myotomal pattern, diagnosed clinically and with MRI, and managed conservatively or surgically if severe
